add emojis- Job Title: Care Manager – Social Work (MUST HAVE LMSW or LCSW along with MSW) Department: Care Delivery Reports To: AVP of Care Delivery Operations PAY $ 43-  45  Job Purpose Responsible for overall care management and quality of care for participants. Reviews assessments, coordinates holistic care plans across all domains, and ensures culturally sensitive, person-centered care. Key Responsibilities Participate in interdisciplinary care planning meetings and represent social work discipline Review documentation for quality; address deficiencies with staff and follow disciplinary policy Monitor, coach, and conduct training sessions for field staff Coordinate care plans with interdisciplinary team (IDT) to meet member needs and goals Arrange and authorize services including ADLs/IADLs support, housing, meals, transportation, and community-based services Coordinate care transitions across settings (home, hospital, rehab, nursing facilities) Collaborate with PCPs, specialists, and IDT on member condition changes and care planning Ensure ongoing assessments, service authorizations, and evaluations are completed Monitor effectiveness of care plans and adjust interventions as needed Document all services per organizational and regulatory standards Maintain fiscal responsibility for service utilization Support development of policies, procedures, and program improvements Participate as active member of IDT Ensure compliance with scope of practice, competencies, and organizational standards Perform other duties as assigned Schedule: 8:00 AM – 5:00 PM (40 hours/week) Qualifications Education: Master's in Social Work (NYSED-approved program required) Experience: Minimum 2 years administrative experience in home health, managed care, acute care, long-term care, or related setting Experience with frail/elderly populations required (or training provided if under 1 year) Supervisory experience preferred Customer service experience required Managed long-term care experience beneficial License: Active, unrestricted New York State Social Work license required Language: Bilingual preferred Health Requirements: Medical clearance and up-to-date immunizations required Physical Requirements Must be able to: Stand up to 6 hours; sit up to 6–8 hours Lift up to 50 lbs and assist with patient mobility Bend, squat, climb stairs, and travel between sites Perform fine motor tasks and operate equipment Communicate clearly verbally and in writing Demonstrate cognitive ability for decision-making and documentation  RESUMES TO :
