Position Responsibilities: The Care Manager, as part of the Nutrition Department, is responsible to assess older adults to determine the need and eligibility for a Home Delivered Meal that will provide nourishment and adequate nutrition for the individual.
Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)
Maintenance of an ongoing caseload including:
- Assessment of an elder's nutritional and environmental status to determine eligibility and appropriateness for a home delivered meal within program timelines and regulations
- Development and implementation of the meal plan, with the elder and formal and informal supports within program guidelines.
- Reassessment and monitoring of consumer's eligibility and needs on an ongoing basis through telephone calls, home visits and case conferences as specified in program guidelines.
- Arrangement for crisis intervention services when necessary.
- Arrangement for a Home Care Referral to assess for other services when necessary.
- Completion and maintenance of up-to-date documentation, including paper case files, computer data entry and statistical records as required by the agency and the Executive Office of Elder Affairs.
- Maintain caseload in compliance with regulations governing Title 3 Home Delivered Meals Programs as set forth by the agency and the Executive Office of Elder Affairs.
- Case coverage during team members' absence as well as during normal business hours to respond to Title 3 Meals consumer's not found at home to receive their meal.
Participation in Staff Development Programs:
- Participate in team meetings, clinical supervision, Home Care and general staff meeting as scheduled.
- Participate in professional development activities to maintain expertise and develop additional skills.
- Assist in the orientation and training of new staff members as needed.
Qualifications: Associates Degree in social work, human services or related field required; experience with elders preferred. Significant relevant work experience may be substituted for portion of degree. Strong organizational, written, verbal and computer skills required. Bilingual ability a plus. Must have valid Driver's License and reliable transportation.
At AgeSpan, we carefully consider a wide range of factors when determining rate of pay. Actual rate of pay depends on factors including but not limited to internal equity, experience, and qualifications. The pay range for this position is $24.50 - $25.50/hour.
