Care Partner/Medication Aide (2:00 PM – 10:00 PM)

Position Summary

Topside Manor is seeking a compassionate, dependable, and team-oriented Care Partner/Medication Aide to provide exceptional, person-centered care within our small-home skilled nursing community. This role blends medication administration with hands-on caregiving, allowing you to build meaningful relationships with the elders who call Topside Manor home.

During the evening shift, the Care Partner/Medication Aide is responsible for administering medications to approximately 15 elders within their assigned home while also working alongside fellow Care Partners to provide personal care, support activities of daily living, and contribute to a warm, comfortable home environment.

Essential Duties & Responsibilities

Medication Administration

  • Safely administer medications and treatments as ordered by the physician and delegated by the licensed nurse.
  • Complete evening medication passes for approximately 15 elders within the assigned home.
  • Observe elders for medication effectiveness, side effects, adverse reactions, or changes in condition and promptly report concerns to the licensed nurse.
  • Accurately document medication administration and related observations in the electronic medical record.
  • Maintain medication carts, storage areas, and documentation in accordance with Kansas regulations and facility policies.
  • Ensure medications are administered safely while honoring each elder's dignity, preferences, and daily routine.

Care Partner Responsibilities

  • Provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, eating, and mobility.
  • Assist with transfers, repositioning, ambulation, and other mobility needs using safe resident handling techniques.
  • Respond promptly and respectfully to call lights and resident requests.
  • Provide companionship, emotional support, and meaningful engagement throughout the shift.
  • Assist with meal service, snacks, hydration, and evening routines.
  • Help maintain a clean, safe, and welcoming home environment.
  • Observe and communicate changes in an elder's physical, emotional, cognitive, or behavioral status.
  • Support individualized care plans while honoring each elder's choices, routines, and independence whenever possible.

Household & Team Responsibilities

  • Work collaboratively with nurses, Care Partners, therapy staff, dietary staff, and leadership to provide exceptional care.
  • Foster a relationship-based approach that promotes dignity, purpose, and meaningful living.
  • Participate in household responsibilities that contribute to the daily life of the home, including light housekeeping, laundry, meal support, and other shared duties as needed.
  • Assist coworkers when needed to ensure all elders receive timely, compassionate care.
  • Promote a positive, respectful, and supportive culture within the home.

Qualifications

  • Current Kansas Certified Nurse Aide (CNA) certification required.
  • Current Kansas Certified Medication Aide (CMA) certification required.
  • Previous experience in long-term care or skilled nursing preferred.
  • Strong communication, time management, and organizational skills.
  • Ability to prioritize medication administration while meeting the care needs of elders throughout the shift.
  • Compassionate, dependable, and committed to person-centered care.

Physical Requirements

  • Ability to lift, transfer, reposition, and assist elders safely.
  • Ability to stand, walk, bend, stoop, and remain active throughout an eight-hour shift.
  • Ability to safely push medication carts and transport supplies throughout the home.
  • Ability to respond quickly and appropriately during emergency situations.
