Care Support Specialist - Position Overview

  • The Care Support Specialist provides intensive case management services to youth and their families such as linking children and families to community resources for aftercare services, scheduling medical appointments for youth while at Cares, and actively advocating for the youth in child and family team meetings.
  • The Care Support Specialist assist with communication between all campus disciplines and helping maintain continuity of care of youth by all employees, functioning as a positive support person for youth and staff.

Job Responsibilities:

  • Provide support to CARES Manager, clinical staff, admissions staff, nursing staff, direct care staff, youth, guardians, and administration by facilitating communication among the different disciplines at CARES Center.
  • Coordinate with Lead RCS for assigned milieu areas weekly for purposes of obtaining information on youth's care/hygiene needs. Follow through with assisting youth as needed (including purchasing care items for youth).
  • Maintain updated schedule of Child and Family Team Meetings, per licensing standards, and notify team of schedule weekly.
  • Coordinate therapeutic passes in conjunction with clinical and medical staff in accordance with licensing standards.
  • Notify pharmacy of therapeutic pass medication needs via fax weekly or as needed
  • Assist with transportation arrangements of youth/families as needed for family sessions, therapeutic passes and visits.
  • Participate in child and family team meetings and document in electronic health record
  • Participate in clinical community meetings as needed and document in electronic health record.
  • Facilitate morning meeting daily.
  • Manage and maintain hygiene closet.
  • Coordinate discharge planning process for youth beginning at admission to ensure continuity of care.
  • Maintain knowledge of available services, resources and supports to youth in their local communities. Identify needed services and link youth to these services during treatment or, at a minimum, prior to discharge.
  • Perform other duties as assigned.

Required Qualifications:

  • Minimum of a bachelor's degree in a behavioral health field or a bachelor's degree and two (2) years of experience in a behavioral health setting.

  • Prefer two years of experience working with emotionally and/or behaviorally disordered youth.

  • Must be able to read, write, speak and understand English, and be able to communicate in a therapeutic and professional manner.
