Case Management

Pro Med Healthcare Services is actively seeking experienced professionals for the Case Management role to join our dynamic healthcare team. The Case Manager will play a crucial role in coordinating patient care and ensuring that patients receive the appropriate services throughout their care continuum.

As a Case Manager, you will be responsible for collaborating with healthcare providers, patients, and their families to develop and implement comprehensive care plans. You will utilize your clinical expertise to assess patient needs, facilitate effective communication among team members, and advocate for patients to ensure they receive the most appropriate and effective care.

Shift time: 8a-4:30p

Estimated Pay Package: $2100 (stipend included)

Key Responsibilities:

• Conduct comprehensive assessments to determine patient needs and establish care plans.
• Coordinate with interdisciplinary teams to deliver high-quality patient care.
• Monitor patient progress and adapt care plans as necessary.
• Assist in discharge planning and ensure appropriate follow-up services are in place.
• Educate patients and families about care options and resources available.
• Document all activities and maintain accurate patient records.

Requirements

Qualifications:

• Current Registered Nurse (RN) license in the state of practice.
• Bachelor's Degree in Nursing (BSN) preferred.
• Minimum of 2 years of clinical nursing experience.
• Acute Hospital RN Case Management experience
• Critical Care
• Prior experience in case management or care coordination desired.
• Strong critical thinking and organizational skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively in a fast-paced environment.
