BecometheCaseManagementDirectoryou'vealwaysaspiredtobe

·Assumeresponsibilityfortheday-to-dayoperationsandhumanresourcemanagementofthe Case Management department.

·Overseetheinterdisciplinaryplanofcareandthedischargeplanningprocesstoensurethe effectivenessandappropriatenessofserviceswithacentralfocusoncensusmanagement, patient care outcomes, and key care indicators.

·Actasapatientandfamilyadvocate,ensuringthatservicesaredeliveredtomeettheneedsof patients and their families.

·ProvideguidanceandsupporttoCaseManagersandotherstaff,includingtrainingonmanaging caseloadsandinterpretingregulations,policies,operationalprocedures,andobjectives.Review operations to ensure a high level of quality consistent with organizational standards.

·Buildrelationshipswithinsurancecompanies,self-insuredemployers,casemanagementfirms, and other healthcare networks.

·Celebratetheaccomplishmentsandsuccessesofourdedicatedemployeesalongtheway.

Qualifications

·CurrentCCM®orACMTMcertificationispreferred.

·Mustbequalifiedtoindependentlycompleteanassessmentwithinthescopeofpracticeof his/her discipline.

·Iflicensureisrequiredforthedisciplinewithinthehospital’sstate,individualmustholdan active license.

·ForNursing,mustpossessbachelor’s degree in nursing(BSN)withRNlicensure.

·Forothereligiblehealthcareprofessionals,mustpossessaminimumofabachelor'sdegree;a graduate degree is preferred.

·Threeyearsofhospital-basedCaseManagementexperience,includingUtilizationReviewand Discharge Planning experience.

·Mayberequiredtoworkweekdaysand/orweekends,eveningsand/ornightshifts.

·Mayberequiredtoworkonreligiousand/orlegalholidaysonscheduleddays/shifts.
