Job Description
The Case Management Extender is responsible for providing administrative and clerical support to the Case Management Director and Case Management Team including assisting with audits, reports, and data collection. Assists in the coordination of daily activities and projects. Participates in data collection projects and compiles data for presentation to Director, Executives and Physicians' Committees. Demonstrates a working understanding of federal and state regulations and departmental policies, particularly those related to discharge planning and utilization review.
Essential Functions
Plans, provides and maintains a safe environment for patients, staff and families.
Provides accurate and consistent documentation of chart reviews and interactions with personnel from insurance companies.
Participates in department and organizational performance improvement activities
Utilizes appropriate problem solving and conflict management skills to manage issues that arise.
Remains alert to changes in the work environment and responds appropriately.
Education
- High School Diploma or GED
Certification
- Basic Life Support (BLS)- American Red Cross or American Heart Association
Work Experience
- Two (2) years' experience in administrative, hospital case management, social service and/or managed care performing discharge planning, interfacing with insurance companies and/or utilization review
