Job Description
Day (United States of America)Case Management SupervisorActs as a patient advocate/case manager to hospital clients. An autonomous role that coordinates, negotiates, procures services and resources for the management of complex patients to facilitate discharge and achievement of quality and cost-efficient patient outcomes. Looks for opportunities to reduce cost while ensuring the highest quality of care is maintained. Provides supervision as acts as a resource to unit-based care management team members. The Case Management Supervisor assists in the identification of appropriate utilization of resources across the continuum of care. Works collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of patient care. The Case Management Supervisor takes an active role in maintaining a high level of functioning in Multi- Disciplinary Rounds, performing audits of various MDR teams, and reporting findings to the Director of Case Management.
Job Requirements
- Bachelor of Science in health care field in nursing, or Masters in a related or complementary field. Consideration will be given to an applicant enrolled in a program with commitment to finish the program in an agreed-upon time
- Medical Terminology – ability to pass exam with 80% accuracy within three months of hire
- Five years’ experience in an acute care setting
- Basic computer skills required
- Accreditation in ACMA desired
- Demonstrates effective interpersonal and communication skills
- Demonstrates flexibility via an ability to adapt to changing priorities
- Demonstrates good customer relations
- Ability to prioritize assignments and effective time-management skills
- Basic knowledge of clinical and psychosocial aspects of patient care
- Must be detail oriented, flexible, and committed to patient advocacy
- Demonstrates skills in planning, organizing, and managing multiple functions and complex processes
- Excellent verbal and written communication skills required
- Knowledge of basic computer software programs
- Knowledge of area community resources and referrals
