Job Description
The Franklin House Transitional Housing Program provides housing for homeless women and their children in a home like atmosphere. The families are each provided a private and comfortable living space in which they reside for approximately 12 months. During this period of time, the women are involved in educational programs, build work-readiness skills, secure employment, gain parenting, financial and other daily living skills. Staff are on-site 24/7 to provide services and support as needed.

Essential Duties and Responsibilities of a Case Manager:
- Assist clients in developing goals oriented towards gaining self-sufficiency, parenting skills, finding permanent housing, etc.
- Provide on-site services and support, including provision of direct service to clients in areas such as money management, daily living skills, psycho-education, work readiness skills, etc., in order to be able to transition into permanent housing
- Provide transportation as needed to assist with child care, grocery shopping and other needs in the community.
- Support families personal outcomes to enhance quality of life
- Utilize an electronic health record (EHR) system to write clear and concise progress notes
- Ability to pass all Agency and program specific trainings
Training:
- All employees are required to complete 10-15 trainings upon hire and on an ongoing basis.
- Trainings are a combination of in person, which require staff to commute to the main office in Raritan NJ, and remote, using Go To Meeting.
- All trainings are paid.

BA & Experience Preferred
Computer skills a must !
Must possess valid NJ/NY/PA driver's license
In order to be eligible for employment, applicant must not have more than 5 points on their driving record and no DUIs within the last 3 years.
