Job Description

Community Care is hiring a Case Manager. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. Community Care's mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. Position located in Ozaukee County. $5000 Sign on Bonus.Case Manager Job Responsibilities:

Manages an assigned caseload of program members in collaboration with an RN Case ManagerCompletes comprehensive member assessments, member centered care plans, & functional screensProvides ongoing care coordination and quality assurance monitoring of servicesMaintains required documentation in adherence to established standards.

Case Manager Job Requirements:

  • Education: Bachelor's Degree in a Human Services or closely related area required. A minimum of three (3) years' experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
  • Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
  • Certification: Eligibility for long-term care functional screener certification is required
  • Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.Case Manager Job Schedule:

Monday to Friday, 8:00 AM-4:30 PM
