Develop and maintain Individual Recovery Plans (IRPs)Provide ongoing case management and care coordination services

Assist individuals in obtaining permanent, supportive, transitional, and emergency housing resources

Connect clients with mental health, medical, substance use, vocational, employment, and educational services

Assist individuals with public benefits, entitlement programs, and financial assistance applications

Coordinate services with behavioral health providers, housing agencies, healthcare organizations, and community partners

Maintain timely, accurate, and compliant documentation

Support individuals in achieving long-term recovery, housing stability, and independence

Required QualificationsEducation & ExperienceBachelor's Degree in Social Work, Human Services, Psychology, Sociology, Counseling, or a related field

Minimum two (2) years of experience in one or more of the following areas:Case ManagementBehavioral HealthMental Health ServicesSocial ServicesHomeless ServicesHuman ServicesCommunity OutreachHousing ServicesSkills & KnowledgeKnowledge of housing resources, supportive housing programs, and public benefit systems

Strong communication, advocacy, engagement, and relationship-building skills

Ability to work independently within a community-based setting

Experience coordinating services across multiple systems of care

Valid driver's license with an acceptable driving record

Preferred QualificationsExperience working with individuals experiencing homelessness or housing instability

Experience supporting individuals living with serious mental illness (SMI) and co-occurring disorders

Knowledge of Housing First, Permanent Supportive Housing, and recovery-oriented service models

Experience with crisis intervention, care coordination, and service navigation

Experience utilizing Motivational Interviewing (MI) and trauma-informed care practices
