- Assist individuals in accessing benefits, financial assistance, housing, and community resources.
- Support individuals in building daily habits, life skills, and achieving recovery goals.
- Facilitate transition planning for individuals moving from inpatient or higher levels of care.
- Provide education and consultation to individuals, families, and clinical teams regarding care planning and case management.
- Contribute to treatment planning, program development, and process improvement initiatives.
- Maintain accurate documentation and records of case management activities.
- Attend weekly team meetings and collaborate with physicians, social workers, nurses, and other clinical staff.
- Arrange transportation through Medi-Cal NMT when needed.
- Travel to meet with individuals in homes, schools, workplaces, and community settings (up to 80% of the time).
- Must have a valid California driver’s license, reliable transportation, and insurance coverage to safely commute to community-based locations.
- Other duties as assigned to support program goals.
- Bachelor’s degree or equivalent experience.
- Minimum one year of case management experience (three preferred).
- Experience working with individuals experiencing psychosis and youth/families preferred.
- Valid California driver’s license, reliable transportation, and ability to be insured.
- CPR/First Aid certification or willingness to obtain.
- Comfortable with travel to community-based locations up to 80% of the time.
- Strong interpersonal skills, cultural sensitivity, and respect for diverse identities.
- Knowledge of California care systems, crisis intervention, and case management best practices.
- Ability to assess needs, coordinate services, and support meaningful recovery goals.
