Case Manager (Bilingual Spanish Required) - Residential Program- Brooklyn, NY (Hybrid) - Contract - $30.00-$33.00 Per HR Title: Case Manager (Bilingual Spanish Required) - Residential ProgramLocation: Brooklyn, NY (On-Site 4 Days and 1 Day From Home)Employment Type: Contract- Flexible, 08:00am-04:00pm, 08:30am-04:30pm, 09:00am-05:00pm, 10:00am-06-00pm (Hybrid)Compensation: Pay Range: $30.00-$33.00 Per HR Great opportunity for anyone considering contract with the chance of direct hireAbout the Opportunity:• Join a respected, mission-driven nonprofit organization that has been serving New Yorkers for generations through housing, behavioral health, refugee and family support, and community-based social services.• This is an excellent opportunity for someone who is passionate about helping vulnerable populations while gaining the potential for long-term employment through a contract-to-hire opportunity.Position Summary:• We are seeking a compassionate and organized Case Manager to support adults residing in a residential housing program. • In this role, you will work directly with clients to help them achieve greater stability, independence, and overall well-being by coordinating services, advocating for their needs, and connecting them with community resources.• The ideal candidate is comfortable working in a fast-paced residential environment, building trusting relationships, and responding calmly during crisis situations.Responsibilities:• Manage an assigned caseload of residential clients while providing ongoing case management and support.• Meet with clients regularly to assess progress, identify needs, and develop individualized service plans.• Coordinate referrals and connect clients with medical, behavioral health, housing, vocational, and community resources.• Advocate for clients and assist with accessing benefits and other supportive services.• Accompany clients to medical, psychiatric, or other essential appointments when needed.• Maintain timely, accurate, and audit-ready documentation, including progress notes, assessments, and service plans.• Facilitate or co-facilitate therapeutic, educational, or life skills groups and encourage resident participation.• Respond appropriately to client crises by utilizing de-escalation techniques and coordinating emergency services when necessary.• Conduct routine room and unit inspections while promoting a safe and supportive living environment.• Assist with daily program operations, including meal service and client engagement activities, as needed.• Maintain strict confidentiality and ensure compliance with agency policies and applicable regulations.• Collaborate with multidisciplinary staff to provide high-quality, person-centered care.• Perform additional responsibilities as assigned to support program operations.Qualifications:• Bachelor's degree required (Social Work, Human Services, Psychology, Sociology, or a related field preferred).• Bilingual Spanish required• Minimum of two years of experience working in human services, residential programs, behavioral health, housing, or a related setting.• Experience supporting individuals experiencing homelessness, mental illness, substance use disorders, HIV/AIDS, or other complex social needs is highly preferred.• Strong interpersonal, crisis intervention, and relationship-building skills.• Excellent written and verbal communication abilities.• Strong organizational skills with the ability to manage multiple priorities.• Working knowledge of HIPAA and client confidentiality standards.• Proficiency with Microsoft Office and electronic documentation systems.• Flexibility to work occasional evenings, weekends, or holidays based on program needs.Why Consider This Opportunity:• Contract-to-hire with the potential for long-term employment.• Competitive pay of $30.00–$33.00/hour.• Opportunity to make a meaningful impact in the lives of individuals experiencing housing instability and other life challenges.• Collaborative, mission-focused team environment with opportunities for professional growth and continued learning.Please email: jisha.thomas@itmmi.com#LI-JT1#INDTWO 26-18013
