Community Care Episodic Case ManagerSisters of Mercy of the Americas – Dallas, PAPay Range: $21–$29/hour

The Community Care Episodic Case Manager supports Sisters of Mercy in meeting their health and wellness needs during periods of illness, recovery, or transition. This consultative role coordinates care, navigates healthcare systems, and ensures Sisters receive compassionate, respectful, and appropriate services.

This is a highly visible, mission-critical role supporting the health and well-being of the Sisters of Mercy during some of their most vulnerable moments. The Community Care Episodic Case Manager serves as a trusted clinical partner, advocate, and coordinator across complex healthcare settings. This position requires strong independent judgment, comfort navigating hospitals and care systems, and the flexibility to respond to changing needs in real time. It is ideal for an experienced healthcare professional who thrives in autonomous, fast-moving, and deeply meaningful work..

Key Responsibilities

  • Provide health and wellness consultation in person, by phone, or at healthcare facilities

  • Coordinate and manage episodic care and services (home care, rehab, nursing care)

  • Assess care needs following illness, hospitalization, or surgery

  • Accompany Sisters and advocate within the healthcare system

  • Collaborate with Sister Life Ministers and health leadership

  • Support transitions to long-term care when needed

  • Respond to emergent health situations as they arise

  • Driving

Qualifications

  • BSN, RN, LPN, Social Worker, Case Manager, or Admissions Coordinator

  • Experience in geriatrics, home care, long-term care, or hospital discharge

  • Strong case management and care coordination skills

  • Ability to work independently with a flexible, as-needed schedule

  • Commitment to holistic, mission-driven care

  • Valid driver’s license and basic computer skills
