Job Description

Case Manager Housing Specialist (Full Time)

Position Summary:

Safe Haven Outpatient Case Manager/Housing Specialist provide services to adults with mental health and addiction challenges to maintain and improve their level of functioning in the community. The Case Manager/Housing Specialist is integral to this goal, working with clients to remove barriers to care. Conducts comprehensive housing assessments and assists clients in finding and maintaining safe, affordable housing.

The Support Includes:

  • Connection to emergency shelter and housing
  • Access basic needs
  • Primary care coordination
  • Access to benefits/entitlements, navigation of legal issues
  • Building improved natural supports and social environment

EDUCATION AND TRAINING REQUIRED:

  • Associate or bachelor's degree in psychology, social work, or a related human services field, with at least one year of relevant experience.
  • Experience with individuals living with mental illness, substance use, chronic health issues, and/or homelessness.
  • Ability to communicate effectively orally and in writing, including documentation as required in an Electronic Health Record.
  • Ability to work as a member of a multi-disciplinary team.
  • Must maintain appropriate boundaries. 
  • Employee will assume full responsibilities for the cost and efforts of maintaining any and all forms of licensure, certification, and credentialing documentation as required in their job description or sited on any Action Form as a qualification for hiring, job promotion, or monetary increase.  Subsequently, the employee will adhere to the credentialing requirements of all insurance/funding sources for which they qualify and provide Human Resources and/or the Credentialing Specialist with the initial required documentation (e.g., licenses, liability insurance policies, DEA numbers, etc.)  and all renewals of these documents thereafter. 
  • Must have a valid driver's license, reliable transportation, and vehicle insurance.

WORKING CONDITIONS:

  • Must demonstrate a genuine positive regard for others and willingness to deliver truly person-centered care, meaning that  is recognized as the expert of themselves.       
  • Services are delivered in a location that best meets the needs of clients and will occur in the community, possibly outdoors, in public settings or at a CCA location.          
  • Effectively accomplishes duties by working with others, including team members, other CCA staff and/or outside agencies.         
  • Regularly available for work, reliable and on time for all things scheduled.         
  • Ability to move within the building to meet with clients, team members or  supplies to attend to client needs.          
  • Ability to transport clients and perform visits  the home and community.         
  • Demonstrates competency in clinical/technical or administrative skills, procedures, methods, and tools required to perform the job successfully.         
  • Able to perform in a  environment and meet or exceed expected service levels, including health home targets.
  • Working from home may be approved by the Supervisor on a limited basis (e.g., car trouble, completing a training, etc.).
