Case Manager –Older Adult CenterQueens, NY Pay: $26–$28/hour Schedule: Monday–Friday (Full-Time)We are seeking a dedicated Case Manager to join a welcoming Older Adult Center in Queens. This role is ideal for someone passionate about supporting seniors through access to services, advocacy, and community-based care.Key Responsibilities:
- Provide information, referrals, and case assistance services (in-person & remote)
- Assist clients with benefits and entitlements (Medicaid, Medicare, public assistance, etc.)
- Coordinate services including home care, transportation, meals, housing, and healthcare
- Conduct support groups and community education sessions
- Maintain accurate case records and reports using electronic systems
- Collaborate with community organizations to support client needs
- Identify and escalate crisis situations to leadership as needed
- Bachelor’s Degree (BSW, BA, or BS required)
- Strong communication and documentation skills
- Computer proficiency and ability to learn internal systems
- Experience in case management or group facilitation preferred
- Bilingual (Spanish, Russian, or Chinese dialects) is a plus
