Job Description

Spurwink is seeking a CCBHC Care Coordinator/Cultural Broker to join the ShifaME team in Cumberland County. This position provides case management services to forcibly displaced children and families experiencing challenges related to resettlement, acculturation, trauma, and isolation. Working across home, school, community, and office settings, the Care Coordinator/Cultural Broker helps families navigate complex service systems, bridges cultural and linguistic differences, facilitates collaboration among providers, and advocates for culturally responsive care. The ideal candidate is collaborative, engaging, and driven, has and possesses the language skills needed to effectively serve diverse communities.This is a full-time, Monday-Friday position with a pay range of $20.00-$26.00 per hour.Duties:Conduct intake, psychosocial, and ongoing assessments to identify client needs and strengths.Maintain accurate and timely documentation, including assessments, service plans, treatment updates, progress notes, managed care entries, referrals, and electronic health records.Facilitate team meetings and develop crisis plans when appropriate.Partner with ShifaME clinicians to provide services using Trauma Systems Therapy for Refugees (TST-r). Collaborate with additional interdisciplinary team members including the Clinical Team Lead, Peer Partner, Nurse Care Manager, and medical and psychiatric consultants.Connect immigrant and refugee children and families with community, educational, medical, behavioral health, and social service resources.Coordinate referrals and serve as a liaison among families, providers, schools, and community partners.Help families navigate complex systems while bridging cultural and linguistic barriers to care.Advocate for culturally responsive, family-centered services and support.Track and report treatment progress to ensure quality, coordinated care.Qualifications:Bachelor's Degree (BA) in a related field required. Must have good computer skills, strong communication skills, and the ability to manage time independently. Candidates must have the linguistic and cultural experience to provide cultural brokering. The ideal candidate will have the language capacity to serve a multicultural population. Fluency in both spoken and written Spanish is preferred.
