Job Description

Position Title: Support Services for Adults Center SpecialistImmediate Supervisor: Support Services for Adults Center Manager

Position Summary:

This position provides support to individuals with developmental disabilities that require protective oversight, behavioral support, or assistance with the activities of daily living (ADL) and certain independent activities of daily living (IADL), serves as a role model and leader within the center, aids the Center Manager with operational duties and assumes a leadership role in the absence of the Center Manager.Pay Rate: $20.50/HrPrimary Responsibilities and Duties:
  • Assumes leadership roles as assigned by the Manager and the Coordinator.

  • Serves as the center contact and oversees center operations in the absence of the Manager and Coordinator.

  • Promotes and models Community Living, Inc. culture mindsets and is an active leader in fostering team cohesiveness and harmony.

  • Coordinates and oversees center safety drills and inspections and completes all associated documentation.

  • Coordinates vehicle upkeep and maintenance; maintains vehicle documentation and submits mileage sheets in accordance with agency requirements.

  • Notifies immediate supervisor of concerns/issues regarding participants, maintenance and repairs in the facility or company vehicles.

  • Completes daily documentation in a timely and thorough manner and in accordance with agency and departmental guidelines.

  • Provides direct support to persons served to include assistance with hygiene, and physical and mobility needs as necessary.

  • Demonstrates familiarity with each program participant’s file by performing activities outlined in the program plan to achieve the goals and expectations identified.

  • Responsible for maintaining the cleanliness of all center areas.

  • Transports participants to and from community outings and actively engages with and supports each person served during all off site activities.

  • Assists Manager and Coordinator with implementing individual and group activities and provides input into each program participant’s plan.

  • Ensures the safety and welfare of all persons served, families and visitors.

  • Passes medications without error as applicable.

  • Completes and maintains all required trainings.

  • Perform all other duties as assigned.

Supervisory Responsibilities:

  • Assumes center leadership role, oversees center operations, and serves as the primary center contact in the absence of the Manager and/or Coordinator.

Qualifications:Education, Experience, Skills:

  • High School Diploma or GED.

  • Two years of experience working with individuals with disabilities.

  • Possess good communication and organizational skills.

  • Possess good listening, comprehension and decision making skills.

  • Must be able to write in a clear, concise professional manner.

  • Ability to work as a team member.

  • Possess excellent computer skills.

Certifications, Licenses, Registrations:

  • Driver’s License

Physical Demands/Working Conditions:

  • Drive a large van to and from events, programs, etc. Must assist persons served entering and exiting the van. Lock down wheelchairs on vans.

  • Physically assists persons served as needed.

  • Lift persons served who cannot bear weight.

  • Push and maneuver wheelchairs.

  • Moderate Work: Exerts up to 50lbs of force occasionally and 20lbs frequently. Standing, sitting, reaching, typing, and walking are common.

  • Family Care Safety Registry Check

  • Motor Vehicle Report (MVR) Check, Proof of Insurance
