Job Description
- Schedule weekly clinical visits with the Director of Clinical Services to facilitate effective patient care and team collaboration.
- Observe, record, and report any changes in patient conditions, including signs and symptoms, the Director of Clinical Services or RN Case Manager as needed.
- Assist in calling and visiting patients to monitor their health and provide support.
- Answer telephone calls, respond to inquiries, and communicate effectively with staff, patients, and families regarding care plans and services.
- Contact outside physicians listed on the hospice assignments upon patient admission and discharge to ensure continuity of care.
- Order DME (Durable Medical Equipment) based on patient condition and RN requests, ensuring timely delivery and setup of equipment.
- Notify DME companies and pharmacies of patient needs and any changes in care requirements.
- Assist team members in coordinating care plans with interdisciplinary team members, ensuring comprehensive patient support and effective communication.
- Participate in team meetings to discuss patient progress and contribute to care strategies.
- Deliver equipment and medication to patients' homes, providing setup and education assistance to ensure proper usage and functionality of the equipment.
- Proficient in Microsoft Windows and Microsoft Office applications.
- Strong organizational, written, and verbal communication skills.
- Demonstrates sound judgment and effective problem-solving using available resources and collaboration.
- Skilled in managing multiple tasks and priorities simultaneously.
- Able to follow both verbal and written instructions accurately.
- Maintains a positive, receptive attitude toward feedback and direction.
- Committed to the hospice philosophy and capable of accurately representing its mission and services.
- Self-driven, able to work independently as well as collaboratively in a team setting.
- Solid understanding of English grammar and general office practices, including typing and filing.
- Experienced in providing quality customer service.
- Basic mathematical proficiency and ability to produce accurate data.
- Familiar with commonly used medical terminology.
- Knowledgeable of Medicare requirements specific to certified hospice programs.
- Minimum Clinical Medical Assistant or Licensed Practical Nurse certification.
- Minimum of 1 year experience in a healthcare environment or equivalent position.
- Operates phone system, computer hardware, calculator, printer, facsimile, copiers and other standard office equipment.
- Frequent use of own personal motor vehicle and cell phone to run errands as directed.
- Primarily Monday through Friday 8:00 a.m. to 4:30 p.m., with a half-hour lunch break.
- Duties may occasionally require after-hour work, including weekends as needed.
- Holidays observed.
