Certified Nursing Assistant (CNA) – Full-Time | Field-Based

Vitality Hospice & Palliative Care is a clinician-owned and operated hospice seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing interdisciplinary team.

In this field-based role, you will provide hands-on care and emotional support to patients in their homes, helping preserve comfort, dignity, and quality of life during their end-of-life journey. Schedule & Position Details

• Full-Time, Monday–Friday
• Typical hours: 7:00 AM – 3:00 PM
• Occasional evening/weekend overtime available
• Field-based role with local travel
• Mileage reimbursement provided
• Hiring immediately

Key Responsibilities

Under the direction of a Registered Nurse Case Manager, the CNA will:

• Assist patients with mobility, transfers, and ambulation
• Provide personal care including bathing, grooming, dressing, and feeding
• Offer companionship and emotional support to patients and families
• Perform light housekeeping tasks related to patient care
• Accurately document care provided in a timely manner
• Follow individualized care plans and hospice standards
• Support a culture of compassion, respect, and patient-centered care

Qualifications

• Active Certified Nursing Assistant (CNA) certification (required)
• Valid driver's license, reliable vehicle, and current auto insurance
• Ability to work independently in the field
• Strong communication, time-management, and organizational skills
• Compassionate, dependable, and patient-focused
• Experience with hospice, palliative care, or chronic illness preferred (not required)
