Role & ResponsibilitiesCompassionate End-of-Life Care: Provide sensitive, resident-centric daily personal care to patients in a long-term care setting, ensuring comfort, dignity, and peace.Patient Hygiene & Comfort: Assist with personal hygiene, grooming, bathing, light nutrition, and repositioning/safe transfers. Measure and record vital signs.Family Support & Communication: Serve as a comforting, professional presence, reporting any clinical changes or urgent needs promptly to the hospice nursing supervisor.Environment Maintenance: Assist with maintaining a clean, calm, and supportive home environment for the patient and their visiting loved ones.Technology Proficiency: Demonstrated ability to navigate MatrixCare efficiently with your cellphone, ensuring timely and accurate documentation of patient care, timesheets, and travel time.Ability to Travel (when needed): Capable of independent travel to various local client residences, when needed, and maintaining detailed and precise records of travel time and destinations.RequirementsMust possess an active Pennsylvania CNA certification
Minimum 6 months of hospice, home health, skilled-nursing, or related clinical experience
