Ultimate Therapy is hiring a full-time Certified Occupational Therapy Assistant to support rehab services in a skilled nursing facility. This role focuses on helping residents improve independence and function through guided, hands-on care.

Responsibilities:

Provide treatment based on plans developed by the supervising therapist
Lead activities that support self-care, coordination, and daily function
Use adaptive techniques and tools to promote safety and independence
Monitor resident progress and communicate updates to the care team
Educate residents and caregivers on carryover strategies
Complete accurate documentation

Qualifications:

Active Certified Occupational Therapy Assistant license required
Team-focused with a strong commitment to resident care
