Chief Operating Officer (COO) – Healthcare / Home Care

Location: Long Island, NY – Nassau, Suffolk, and Queens Counties, NYSalary: $215,000–$230,000 Base + BonusEmployment Type: Full-Time, PermanentRelocation Assistance: May be available

Position Overview

We are seeking an experienced Chief Operating Officer (COO) to join a rapidly growing, established home care organization serving the New York City and Long Island markets. This is a newly created executive leadership position with a company generating approximately $20 million in annual revenue and expanding through strategic acquisitions. The COO will work directly with the CEO to oversee operations across multiple locations, strengthen leadership teams, improve operational performance, and develop the infrastructure needed to support continued growth. This opportunity is ideal for a hands-on healthcare executive who thrives in an entrepreneurial environment and wants to make a measurable impact on a growing organization.

Key Responsibilities

  • Oversee day-to-day operations across multiple home care locations throughout the NYC and Long Island markets.

  • Provide direct leadership and oversight to existing Directors of Operations.

  • Develop and implement strategies to improve operational efficiency, profitability, and business growth.

  • Establish and monitor key performance indicators (KPIs) to drive accountability and results.

  • Build scalable processes, systems, and organizational structures to support expansion.

  • Lead, mentor, and develop management teams while fostering a high-performance culture.

  • Identify opportunities to leverage technology, automation, and AI to improve operations.

  • Partner closely with the CEO on strategic planning, organizational development, and long-term growth initiatives.

  • Support integration and operational alignment following acquisitions.

Qualifications

  • Significant senior-level operational leadership experience in healthcare, home care, home health, or a related service-based industry.

  • Proven ability to manage multiple departments, locations, and leadership teams.

  • Demonstrated success scaling operations and driving measurable business improvements.

  • Strong understanding of financial performance, KPIs, process improvement, and operational strategy.

  • Experience building accountable, high-performing teams.

  • Ability to work independently and make strategic and operational decisions in a growing organization.

  • Strong communication, leadership, and problem-solving skills.

  • Home care or home health experience strongly preferred.

  • MBA preferred.

Compensation & Highlights

  • Base Salary: $215,000–$230,000 annually

  • Bonus: Additional performance-based bonus opportunity
