Summary of Position: As a part of the Santo Domingo Pueblo's Kewa Family Wellness Center, the CHW (Community Health Worker) Transporter primarily focuses on providing transportation services to community members within a tribal or rural healthcare setting. This role involves safely transporting individuals to and from medical appointments, ensuring their comfort and well-being during the journey, and coordinating with healthcare providers. Additionally, CHW transporters may assist with patient care, such as helping with mobility or basic needs, and advocating for their clients' health needs.
Minimum Qualifications:
- High School Diploma or GED equivalent.
- Knowledgeable of a holistic approach to individual and Community Health legislation, theories, practices and procedures as well as traditional medicine and medical terminology.
- Indian Health Service (IHS) Basic Community Health Representative (CHR) Certification. Must obtain within the first 3 months of employment.
- State of New Mexico Community Health Worker (CHW) Certification. Must obtain within the first 3 months of employment.
- CPR & First Aide Certified or obtain within the first 30 days of employment.
- Indian Health Service Confidentiality training (HIPPA and Private Act Policy) or obtain within the first 3 months of employment.
- Fluent in Keres language is preferred and knowledgeable of Santo Domingo Pueblo's cultural values, norms and traditions.
- Must be able to stand, bend, and walk within the community, schools, clinics, and homes.
- Ability to lift 50 lbs. when necessary to set up equipment for special events, clinics, and teaching/outreach programs.
- Must have ability to translate written or verbal medical/health education information in English into Keres and vice versa.
- Extended working hours, including weekends, may be required.
- Must be able to satisfactorily pass a background check.
- Valid New Mexico driver's license with ability to meet Santo Domingo Pueblo liability insurance requirements and maintain eligibility for insurance.
- Must be 21 years of age or older for insurance purposes.
