Job Description:

Position Summary and Purpose

The Admission Coordinator works with staff at referring facilities to identify and assess potential patients for rehab services; Evaluates the medical records of potential patients to ensure medical necessity for acute rehab; Acts as the primary liaison between the patient, referring facility, physicians, payers and Frazier Rehab Institute; Prepares, educates and otherwise facilitates transitions for patients from acute care to the rehab setting; Ensures any special needs of the patient are properly arranged for prior to the patient's admission; Provides necessary insurance information on potential patients to Insurance Verification and/or Financial Counselor and obtains all necessary precerts prior to a patient's admission; Gathers clinical and non-clinical patient information to complete a required online Preadmission Assessment for all incoming patients; Conducts or coordinates informational/marketing programs to referral sources as needed to maximize referrals to Frazier. Essential Functions:

  • Assists in delivering high quality health care services in a professional, compassionate, and courteous manner while respecting the dignity and individuality of each person who comes in contact with the organization by following the Standards of Excellence, the Standards of Behavior, and the Standards of Professional Appearance and Conduct.   • Knowledge and performance of Code of Conduct Standards is evident in performing daily activities. (Mandatory for all employees)

  • Work reflects the employee honors the Frazier Rehab Institute's Mission Statement, Vision Statement, and Corporate Values. (Mandatory for all employees)

  • Demonstrates the knowledge and skills necessary to interact with and provide care appropriate to the age of the patients served. (Mandatory on all patient care positions)

  • Works with appropriate staff at referring facilities (e.g. physicians, discharge planners) to establish and maintain referral sources.

  • Evaluates the medical records of potential patients and identifies rehab potential according to criteria established by the Medical Staff.

  • Accurately presents the condition of incoming patients to hospital staff.

  • Educates and prepares the patient and/or the family for admission.

  • Explores and secures financial coverage for services.

  • Plans and organizes the programs and operations of the department within the Hospital's strategic plan and mission statement.

  • Provides for services and solves problems relating to efficient operations of the department

Other Functions:

Maintain compliance with all company policies, procedures and standards of conduct.

Performs other duties as assigned.

Job Requirements

(Education, Experience, Licensure and Certification)

Education:

  • Kentucky RN license (Associate or Bachelors Degree) or other licensed clinical professional

  • Experience:

  • Five years minimum of nursing/licensed clinical experience working with a broad base of patient types/diagnosis.

  • Previous rehabilitation experience required.

  • Knowledge of Medicare, Medicaid, and Third Party Payer systems also required.

  • Preferred:

  • Experience with patient/family counseling and problem resolution preferred.

  • Marketing and sales background also preferred.

Certification:

  • Basic Life Saving (BLS) accredited by the American Heart Association (AHA) (required)

Job Competency:

  • Knowledge, Skills, and Abilities critical to this role:

  • Excellent psychosocial assessment skills

  • Short term crisis intervention skills

  • Understanding of medical terminology

  • Previous insurance experience

  • Effective organizational skills and ability to multi-task.

  • Effective at building and maintaining internal and external relationships that support department mission.

  • Computer skills for applications noted below.

  • Language Ability:

  • Must be able to communicate effectively in both verbal and written formats.

  • Reasoning Ability:

  • Ability to break down problems or tasks; scanning one's own knowledge and experience to identify causes and consequences of events.

  • Computer Skills:

  • Excellent ITW, Microsoft Office, Excel, and PowerPoint, Adobe skills.
