Job Title: LTSS Service Care Manager
Location: Kent County, DE
Duration: 6 Months (possibility of extension and conversion)
Schedule: Mon-Fri 8am 5pm EST
PR: $40 to $41/hr.
Position Summary
The LTSS Service Care Manager assists in developing, assessing, and coordinating holistic long-term services and supports (LTSS) care management activities to achieve quality, cost-effective healthcare outcomes. This role supports members by developing individualized service care plans, coordinating care, conducting assessments, and educating members and caregivers on available services and benefits.
This position is member-facing and includes home and site visits within the assigned county.
Key Responsibilities:
- Evaluate member needs, available resources, and recommend or facilitate care plans to achieve optimal outcomes
- Develop, monitor, and update long-term care service plans based on assessments and ongoing member needs
- Coordinate care between members, caregivers, providers, specialists, and community resources
- Conduct home and site visits to assess member needs and collaborate with healthcare partners
- Support LTSS level-of-care redeterminations and ongoing care management activities
- Provide education to members and caregivers on services, benefits, referrals, and care instructions
- Assist members with access to resources such as housing, employment, independent living, justice, and participant-directed services
- Monitor member outcomes and recommend care plan adjustments as needed
- Maintain accurate documentation to ensure compliance with state, federal, and payer regulations
- Communicate effectively with healthcare providers and community partners
- Provide feedback to leadership on opportunities to improve care quality and service delivery
- Perform other duties as assigned and comply with all organizational policies and standards
Typical Day in the Role:
- Monday Friday, 8:00 AM 5:00 PM
- Conduct member home visits and assessments
- Complete documentation and care plan updates
- Collaborate with the LTSS case management team
Qualifications:
Required:
- Bachelor's degree and 2 4 years of related experience
- Delaware or Compact RN License
- Valid driver's license and ability to travel within assigned county
Must have:
- Driver's license, home workspace | Level of Care Redetermination, Transition of Care and Case Management experience
Performance Indicators:
- 4-5 assessments per week, for redeterminations 4-5 daily
Skills:
- Computer literate
- Experience on evaluating older adults/disabled level of functioning
- Experience with medication reconciliation
