Job Description

Make a Difference Through Advanced Clinical Pharmacy Practice

Join a collaborative healthcare team dedicated to delivering exceptional patient care. As a Clinical Pharmacist, you will provide comprehensive pharmaceutical care through integrated medication management, clinical services, and drug information support. Working closely with physicians, nurses, pharmacy technicians, and other healthcare professionals, you will play a critical role in ensuring safe, effective, and evidence-based medication therapy for patients of all ages.

What You'll Do

  • Review, verify, and dispense medication orders in accordance with established policies, procedures, and protocols.

  • Oversee pharmacy personnel in the preparation and distribution of medications.

  • Ensure compliance with USP 797 standards for sterile compounding.

  • Monitor medication therapies for safety, effectiveness, allergies, contraindications, drug interactions, and appropriate dosing.

  • Participate in antimicrobial stewardship initiatives and medication management protocols.

  • Conduct medication histories and support medication reconciliation efforts.

  • Provide timely and accurate drug information to physicians, nurses, patients, and families.

  • Educate staff and patients on medication therapies and best practices.

  • Document clinical interventions and pharmacy activities accurately.

  • Detect, report, and help prevent adverse drug events and medication errors.

  • Support formulary management and medication-use optimization initiatives.

  • Mentor pharmacy staff, residents, students, and interns.

  • Participate in quality improvement projects, department meetings, and organizational initiatives.

  • Promote a culture of patient safety, collaboration, and continuous improvement.

Minimum Qualifications

Education

  • Doctor of Pharmacy (PharmD) degree or equivalent combination of education and experience required.

Experience

Candidates must meet one of the following:

  • Completion of a PGY-1 Pharmacy Residency, OR

  • Board Certification (BCPS), OR

  • More than 5 years of experience with at least 50% of practice in a clinical pharmacist role.

Licensure

  • Maryland Pharmacist License required or eligibility to obtain licensure.

  • Pharmacists obtaining licensure through reciprocity or recent graduates must secure Maryland licensure within 4 months of hire or graduation, as applicable.
