Position Summary

Clinical Pharmacist

Baldwin Family Health Care is seeking a passionate and patient-centered Clinical Pharmacist to join our growing healthcare team. This unique opportunity combines direct patient care, chronic disease management, interdisciplinary collaboration, and pharmacy residency leadership.

The Clinical Pharmacist will work closely with providers and care team members to optimize medication therapy and improve patient outcomes through evidence-based pharmaceutical care. The successful candidate will also serve as the primary preceptor for the PGY1 Community-Based Pharmacy Residency Program's Patient Care Learning Experience and function as the Residency Program Coordinator (RPC).

Key Responsibilities

Clinical Pharmacy Services

  • Collaborate with providers and healthcare staff to deliver evidence-based, cost-effective medication management.
  • Conduct comprehensive medication reviews and medication reconciliation.
  • Provide patient education on medications, disease states, lifestyle modifications, and self-management strategies.
  • Identify, resolve, and prevent medication-related problems.
  • Respond to drug information questions from providers and staff.
  • Monitor laboratory values and clinical outcomes to optimize medication therapy.

Ambulatory Care & Chronic Disease Management

Under Collaborative Practice Agreements, manage patients with chronic conditions including:

  • Diabetes
  • Hypertension
  • Hyperlipidemia
  • Anticoagulation
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Tobacco Use Disorder

Program Development & Quality Improvement

  • Assist in the development, implementation, and evaluation of clinical pharmacy services.
  • Participate in quality improvement and population health initiatives.
  • Support medication access programs and cost-containment strategies, including 340B opportunities.

Residency Program Leadership

  • Serve as Residency Program Coordinator for the PGY1 Community-Based Pharmacy Residency Program.
  • Act as primary preceptor for the Patient Care Learning Experience.
  • Mentor pharmacy residents and contribute to residency program development and accreditation activities.
  • Promote a culture of teaching, learning, and professional growth.

Qualifications Required

  • Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
  • Licensed Pharmacist in the State of Michigan or eligible for licensure.
  • Outpatient, community, ambulatory care, or retail pharmacy experience.
  • Strong communication, organization, and interpersonal skills.
  • Ability to work effectively in a multidisciplinary team environment.
  • Commitment to serving diverse and underserved patient populations.
  • Completion of a PGY1 Pharmacy Residency, and/or ambulatory care practice experience.

Preferred

  • Experience working within a Federally Qualified Health Center (FQHC) or similar safety-net healthcare setting.
  • Experience with the 340B Drug Pricing Program.
  • Familiarity with PioneerRx and NextGen electronic health record systems.
