Job Details / Responsibilities:
Provide clinical pharmacy services in an inpatient hospital settingReview and verify medication orders for appropriatenessCollaborate with physicians, nurses, and interdisciplinary teams on patient care plansMonitor patient therapy for efficacy and safety, including pharmacokinetic dosingParticipate in code responses and emergency situations as neededEducate patients and staff on medication use and safetySupport hospital formulary management and medication safety initiativesWork a mix of day and evening shifts with every other weekend rotation
Qualifications:
Doctor of Pharmacy or BS Pharmacy degree requiredMinimum of 2 years of pharmacist experienceHospital experience preferredPharmacy state licensure required within 60 days of start dateBLS or ACLS requiredPALS required for pediatric practice
