Job Description:

Supports quality improvement, clinical programs, and patient care initiatives through chart abstraction, data collection, and other administrative tasks

Responsibilities:

• Compare medical chart notes with HEDIS/PQA/IHA measure specifications and guidelines to determine if documentation meets requirements for quality measures

• Abstract clinical information from paper/electronic medical records and input data accurately into a quality platform to improve measure performance

• Support medication reconciliation post-discharge efforts by sending faxes and calling provider offices/facilities for information

• Pull and prep weekly appointment Gaps in Care reports for the Virtual Pharmacy team

• Obtain medication lists from provider offices for pharmacist review

• Assist with scheduling pharmacist consultations

• Pull Likelihood of Hospitalization (LOH) reports and share them with provider offices

• Directly outreach to patients to assist in closing gaps in care

Skills and Experience Required:

• Bachelor's degree or equivalent experience

• 3+ years of experience in healthcare industry

• 1+ years of clinical chart review/data abstraction experience preferred

• Basic electronic medical record knowledge and ability to quickly learn new software

• Ability to organize tasks and complete projects in a timely manner

• Knowledge of quality measures (ie, HEDIS)

• Excellent interpersonal, verbal and written communication skills

• Attention to detail and ability to follow precise instructions

Additional Information:

Salary: $20 - $23 Hourly
