Clinical Team Specialist (RN)

What You'll Do

  • Provide clinical oversight of patient care services to ensure quality outcomes and compliance.
  • Coordinate patient care and referrals while ensuring patient needs are continuously assessed.
  • Support the development, implementation, and ongoing evaluation of individualized patient care plans.
  • Review and approve plans of care, ensuring clinical appropriateness and regulatory compliance.
  • Conduct regular OASIS reviews and provide guidance on documentation accuracy.
  • Ensure required patient visits are completed timely and according to established standards.
  • Review clinical documentation for completeness, accuracy, and adherence to state and federal regulations.
  • Monitor the timely submission of required documentation through Homecare Homebase (HCHB).
  • Maintain accurate clinical records in accordance with licensing, accreditation, and regulatory requirements.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives and strategic planning activities.
  • Collaborate with interdisciplinary team members to support exceptional patient care.
  • Perform additional duties and special projects as assigned.

Qualifications

  • Associate degree in Nursing required; BSN preferred.
  • Minimum of two (2) years of home health experience as a Registered Nurse.
  • Current, unrestricted RN license in the state of employment.
  • Strong knowledge of home health regulations, OASIS, and clinical documentation requirements.
  • Ability to quickly learn and effectively utilize systems such as Homecare Homebase, Workday, and Microsoft Office applications.
  • Excellent organizational, communication, and problem-solving skills.
  • Ability to travel occasionally to support field offices during audits, operational needs, or special projects.
  • Valid driver's license, reliable transportation, and current auto insurance in accordance with state requirements.

Physical Requirements

  • Ability to frequently lift, push, pull, and support up to 50 pounds.
  • Ability to assist with patient positioning, transfers, and movement of equipment as needed.
  • Ability to work effectively in a fast-paced, clinically focused environment.
