Certified Nurse Aide (CNA) Home Health & Help at Home

Part-Time | 10-15 Hours per Week | Days & Evenings Visiting Nurses Association is seeking a compassionate Certified Nurse Aide (CNA) to join our Home Health & Help at Home team. This role is ideal for individuals seeking flexible, part-time hours while providing high-quality, one-on-one care in patients' homes.

Position Summary

The Home Health & Help at Home CNA is responsible for providing personal care and supportive services in accordance with the individualized Plan of Care, agency policies, and applicable state and federal regulations.

Essential Duties & Responsibilities

  • Provide personal care services including bathing, dressing, and grooming as outlined in the Plan of Care
  • Assist patients with activities of daily living (ADLs) while encouraging independence
  • Demonstrate safe lifting, turning, and repositioning techniques to family members
  • Support families in maintaining normal daily routines and quality of life
  • Observe and report changes in patient condition or incidental needs
  • Maintain accurate documentation in compliance with agency, state, and federal regulations
  • Follow direction from the Case Manager and nursing leadership

Qualifications

  • High school diploma or GED preferred but not required
  • Certified Nurse Aide (CNA) in good standing - required

Schedule

  • Part-time: approximately 10-15 hours per week • Day and evening shifts
