Job Summary: Tryfacta is seeking a CNA for our client in Burlington, VT 05401. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now! Position Title: CNA Location: Burlington, VT 05401 Duration: 13 weeks (Possible extension) Work Schedule: Variable Days Responsibilities for this position include, but are not limited to:

• Provide direct resident care in a long-term care (LTC), rehabilitation, and dementia care setting .
• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobility assistance.
• Obtain and document vital signs and report significant changes in resident condition to licensed nursing staff.
• Assist with transfers, ambulation, repositioning, restorative care activities, and fall-prevention measures.
• Maintain accurate and timely documentation in the PointClickCare (PCC) electronic medical record system.
• Support resident comfort, dignity, safety, and emotional well-being at all times.
• Collaborate with nurses, therapists, CNAs, and the interdisciplinary team to implement individualized care plans.
• Respond promptly to resident needs and changes in condition.
• Follow infection prevention, safety, HIPAA, and facility policies while providing compassionate resident-centered care.

To be considered for this position, you should have: [Skills, Education, or Experience]

• Active Vermont LNA/CNA license required.
• Minimum 1 year of recent LTC, Skilled Nursing Facility (SNF), Rehabilitation, or Dementia care experience required.
• Minimum 1 year of verifiable travel experience required.
• Current BLS certification required.
• Experience with PointClickCare (PCC) or similar EMR system preferred.
• Strong communication, documentation, teamwork, and resident-care skills.
• Ability to work 8-hour shifts only ; 12-hour shifts are not available for this assignment .
• Ability to work a full-time schedule of 40 hours per week for the duration of the contract.
• Candidates must be available for the scheduled start date and orientation cycle.
