What you'll do:
- Help people and their families in their homes or in the community based on their specific needs.
- Be flexible with your schedule to meet the needs of each person and family.
- Drive and go with individuals to community activities when needed.
- Write and turn in notes that describe what activities you did and how they helped the person.
- Follow each person’s support plan and document their progress.
- Keep all required paperwork up to date and organized.
- Keep your car, insurance, and driver’s license information current and on file.
- Help with light cleaning if needed, such as vacuuming, dusting, or doing laundry.
- Be respectful and work well with coworkers, supervisors, and families.

What's required of you:
Understanding or willingness to learn person-centered support philosophies.
Commitment to treating individuals and families with dignity, respect, and privacy.
Ability to work independently in home settings and collaboratively with families and partner agencies.
High School Diploma or GED required.
Must be at least 18 years of age.
Valid Pennsylvania driver’s license and reliable vehicle required.
Experience working with individuals with disabilities preferred.
Ability to sit 3–7 hours per day; stand and walk 1–3 hours per day.
Occasional lifting or moving of items weighing 10–15 pounds.
Occasional twisting, bending, squatting, or kneeling.
Ability to drive within a 15-mile radius.
