- Carries out tasks that are needed to execute the medical and support service plans
- Accompanies patients to appointments when required, provides coaching to patients, delivers monthly or weekly health education encounters
- Maintain client-related records and other required documentation according to the protocols and standards
- Providing education
- Providing one-to-one peer support
- Assisting with support and socialization groups
- Assisting in data collection and data entry
- Completing program paperwork
- Making referrals to internal and external programs jointly with case manager
- Other tasks as assigned
