The Community Health Worker is responsible for assessment, planning and implementing care strategies that are individualized by patients and directed toward the most appropriate, least restrictive level of care. They also Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services, and manage the care plan throughout the continuum of care as a single point of contact for the member. As a Community Health Worker (CHW), you will act in a liaison role with Medicaid/Medicare members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. The coordinator also addresses social determinants of health such as transportation, housing, and food access.

Primary Responsibilities:

  • Assess, plan, and implement care strategies that are individualized by members
  • Create a positive experience and relationship with the members
  • Utilize both company and community-based resources
  • Collaborate with members, family, and healthcare providers
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for members and families as needed to ensure the members' needs are addressed appropriately
  • Document all member assessments, care plan and referrals provided
  • You'll need to be flexible and adaptable in all types of situations
  • Engage members either face to face or telephonically
  • Adhere to detailed, specific documentation requirements in the member's health record
  • Partner with care team (community, providers, internal staff) \Knowledge and continued learning of community cultures and values\May conduct Health Risk Screener (HRS) as needed
  • Provide member education on community resources and benefits
  • Utilize strong skill sets of managing multiple tasks at a time, being self-motivated, driven toward quality results, managing time well, being very detailed oriented and organized, working well in a team and on your own, and ability to manage multiple deadlines
  • Performs other duties as assigned

Required Qualifications:

  • High School Diploma/GED
  • 3+ years of experience working within the Community Health setting within a healthcare role
  • Intermediate level of computer proficiency including Microsoft Outlook, Teams, Excel, Word, etc. and the ability to use multiple web applications
  • The ability to travel in assigned regions to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or provider offices not to exceed 25% of your time and only during regular business hours.
  • Must reside in Nebraska
  • Valid Nebraska driver's license and current automobile insurance with access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
  • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Preferred Qualifications:

  • Experience with Medicaid and Medicare populations
  • Experience with electronic charting and HIPAA
  • Experience with arranging community resources
  • Experience working in Managed Care

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.00 to $36.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
