As a Community Resource Specialist (CRS) at One Health, you'll be a frontline advocate for patients, connecting them with vital resources and support systems. This patient-facing role is essential in helping our most vulnerable community members overcome barriers to care and improve their overall wellbeing.

Type: Full-Time (40 hours/week) - Monday through Friday 8am-5pm with a 1-hour lunch

Salary: $18.91-$19.84/hour DOE

Location: Hardin, MT

Key Responsibilities:

  • Coordinate Care: Facilitate warm hand-offs and referrals between patients, providers, and community services.
  • Screen & Support: Conduct SDOH screenings (PRAPARE) and help patients set goals to overcome barriers.
  • Educate & Advocate: Assist with applications for coverage, benefits, and other essential services.
  • Document Thoroughly: Maintain detailed electronic patient records, tracking outcomes and service plans.
  • Engage the Community: Participate in outreach events and consortiums to promote One Health services.
  • Navigate Resources: Stay current on eligibility rules, insurance options, and enrollment procedures.

What We're Looking For:

  • Strong computer and documentation skills
  • Ability to manage multiple priorities with remote supervision
  • Professionalism, empathy, and clear boundaries
  • Community involvement or connection to the population served
  • High School Diploma or GED
  • Valid Driver's License

Certifications (upon hire or within first year):

  • BLS Certificate
  • Community Health Worker Certificate
  • Certified Application Counselor (CAC)
