COTA - Skilled Nursing Facility | Travel or Permanent

Blue United Sourcing has two openings for experienced Certified Occupational Therapy Assistants (COTAs) at a skilled nursing facility in San Jose, CA. Both positions are available for an ASAP start and open to travel, permanent, or travel-to-perm candidates. Silicon Valley is calling — two spots available.
Details of the role:
- Anticipated start date: Flexible
- Salary range: Up to $1,800 weekly
- Anticipated duration of contract: 13 weeks
- Contract type: Travel, Permanent, or Travel-to-Perm
Clinical Duties:
- Assist with evaluations and implement OT treatment plans.
- Provide therapy to improve residents’ ADLs and functional mobility.
- Support interventions for orthopedic, neurological, and post-acute conditions.
- Teach adaptive techniques and proper use of assistive devices.
- Collaborate with OTs, nursing, and rehab teams on care goals.
- Document therapy sessions, progress, and daily notes accurately.
- Monitor resident responses and report outcomes to supervising OT.
Requirements:
- Current COTA license in the state of California.
- Associate’s or higher degree in Occupational Therapy Assistant from an accredited program.
- Ability to follow OT treatment plans and assist with interventions.
- Strong communication and teamwork skills for interdisciplinary collaboration.
- Accurate documentation and time management skills.
- Commitment to following facility policies and state regulations.
