Location: Porterville, CA
Employment Type: PRN / Per Diem
Pay Range: $40.00 - $45.00 per hour (BOE)

Gateway Post Acute is a premier skilled nursing and post-acute rehabilitation facility in Porterville, CA. We are currently seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team on a PRN basis.
This position offers flexibility while allowing you to deliver meaningful, hands-on care that improves residents' independence and quality of life.
General Purpose
The Certified Occupational Therapy Assistant (COTA) assists the Occupational Therapist in providing direct patient care, implementing treatment plans, documenting patient progress, and supporting residents in achieving functional independence in accordance with physician orders, state practice acts, and facility policies.
Essential Duties and Responsibilities
Treat patients as directed by the supervising Occupational Therapist
Implement treatment plans in accordance with physician orders and established goals
Document daily treatment notes and progress per state and facility requirements
Communicate patient progress, challenges, and treatment responses with the OT and interdisciplinary team
Participate in patient care conferences, rehab meetings, and discharge planning as needed
Educate patients and caregivers on therapeutic activities, adaptive techniques, and safety
Assist with training of Restorative Aides as appropriate
Maintain clean, organized, and safe treatment areas
Report equipment issues promptly to ensure safe operation
Maintain professional and positive relationships with patients, families, and staff
Supervisory Responsibilities
- This position does not have supervisory responsibilities
Qualifications
Education & Experience
Graduate of an accredited Occupational Therapy Assistant program
Skilled Nursing Facility or post-acute experience preferred
Licensure & Certifications
Current, active California Certified Occupational Therapy Assistant license in good standing
Must meet all continuing education requirements per state regulations
Skills
Proficient in electronic documentation systems (PointClickCare and/or Casamba preferred)
Strong communication, organizational, and patient-centered care skills
Ability to work independently and collaboratively within a rehab team
Work Environment
Skilled Nursing Facility / Post-Acute Rehabilitation
Geriatric and short-term rehab population
