Job Description
Duties:
- Collaborate with patients/residents and their families to provide support and resource information.
- Respond to issues identified by patients/residents and their families.
- Facilitate patient/resident transfer within the nursing center to ensure a seamless transition and adjustment.
- Document patient/resident transfers and adjustments accordingly.
- Participate in the health care decision-making process under the direction of the Social Services Director
- Assist residents to and from needed outside physician appointments.
Requirements:
- High School diploma required.
- Associate degree in human services preferred.
- Minimum of two years' experience in long-term care.
- CPR certification required.
