RN and Certified Diabetes Care and Education Specialist (CDCES), LVPP
Valley Health Partners, Allentown PA
Job Summary: Completes diabetes assessments/evaluations, and develops and provides education and support to patients with diabetes. Provides comprehensive self-management education to include glucose monitoring, medication, nutrition, and prevention of diabetes-related complications. Must have experience and knowledge of continuous glucose monitoring devices and insulin protocols. Provides diabetes competency education to staff members and clinicians.
Minimum Qualifications: RN and Certified Diabetes Care and Education Specialist (CDCES) credentialing required.
Preferred Qualification: More than five years of experience in providing diabetes care and support in a clinical setting.
Licensure: RN, CDCES
Skills Needed:
- Ability to use good judgement to respond to emergencies
- Ability to establish program goals and objectives that support the patient's treatment plan
- Knowledge of endocrinology, pharmacology, and emerging technologies in diabetes care
- Ability to participate in quality improvement processes and projects to improve diabetes-related practice metrics
- Highly organized with strong attention to detail
- Strong problem solving and critical thinking skills
- Ability to work with a diverse patient population
Essential Functions:
- Assesses and identifies patient health needs to manage diabetes
- Demonstrate expertise in various diabetes technologies and pharmaceuticals
- Completes medical record documentation in timely manner
- Performs other duties as assigned
