Job Summary

The Mass General Brigham Medical Group is a system-led operating entity formed by Mass General Brigham to deliver high quality, low cost, innovative community-based ambulatory care. This work stems from Mass General Brigham's unified system strategy to bring health care closer to patients while lowering total health care costs. The Medical Group provides a wide range of offerings, including primary care, specialty care, behavioral and mental health, and urgent care, both digitally as well as at physical locations in Massachusetts, New Hampshire, and Maine. Wentworth Health Partners (WHP), an affiliate of Wentworth-Douglass Hospital and member of Mass General Brigham Medical Group, offers Diabetes Care through a comprehensive program designed to support individuals across all stages of diabetes management, including type 1, type 2, prediabetes, and gestational diabetes. This multidisciplinary team, located in our state-of-the-art building at 10 Members Way in Dover, NH, is comprised of board-certified endocrinologists, certified diabetes educators, registered dietitians, and specialized nursing staff who collaborate to deliver personalized care plans. Services encompass individualized and group education sessions focusing on blood glucose monitoring, nutritional counseling, exercise programs, and medication management. In addition to clinical services, the hospital provides community outreach through educational classes, support groups, and seasonal camps for children with diabetes. The program emphasizes a patient-centered approach, aiming to empower individuals with the knowledge and tools necessary for effective self-management and improved quality of life.

We are seeking a part-time, 30-hour Diabetes Educator to join our Diabetes Services team. The weekly schedule will be Monday, Tuesday, and Thursday from 7:45 a.m. and 5:45 p.m.

The ideal candidate is a Registered Dietitian (RD) with a Certified Diabetes Specialist (CDS) credential, who has hands-on experience with insulin pump therapy and continuous glucose monitoring, and will collaborate closely with two other CDEs and the full endocrinology team.

Responsible for providing comprehensive education and support to patients with diabetes, promoting self-management skills, and enhancing health outcomes. This role involves working collaboratively with healthcare teams to develop individualized care plans and facilitate diabetes management programs.

Essential Functions:

  • Conduct assessments to determine the educational needs and readiness of patients to learn about diabetes management.
  • Develop and implement individualized education plans addressing diabetes self-management, including medication management, nutrition, physical activity, and blood glucose monitoring.
  • Facilitate group education classes on diabetes management, prevention, and lifestyle modifications.
  • Create educational materials and resources to enhance patient understanding and engagement.
  • Provide ongoing support and counseling to patients and their families, addressing the emotional and psychological aspects of living with diabetes.
  • Encourage patient adherence to treatment plans and empower them to actively participate in their health.

Associate's Degree Related Field of Study required

Bachelor's Degree Related Field of Study preferred

Master's Degree Related Field of Study preferred

Licenses and Credentials

Certified Diabetes Educator [CDE/CDCES] - Certification Board for Diabetes Care and Education required

Experience

Experience in health and nutrition related to diabetes education, counseling, or related role 2-3 years required

  • Strong knowledge of diabetes pathophysiology, management strategies, and treatment options.
  • Excellent communication and interpersonal skills to effectively educate and support patients and families.
  • Ability to work collaboratively within a multidisciplinary healthcare team.
  • Proficiency in using educational tools and technology to enhance patient learning.
