DEPARTMENTAL SUMMARY: The St. Charles Health System's Clinical Nutrition and Diabetes Department provides services in the inpatient, outpatient, and community setting.

POSITION OVERVIEW: The Certified Diabetes Educator at St. Charles Health System participates as a member of the multi-disciplinary patient care team to provide diabetes education to meet the needs of identified patient population, nurses, and providers. This position does not directly manage any other caregivers.

ESSENTIAL FUNCTIONS AND DUTIES

Performs comprehensive assessment of diabetic patients including physical, psychological, literacy, cultural, and religious influences.

Provides education for 1:1 counseling as well as group classes, and follows-up with patients.

Services in both the inpatient and outpatient setting.

Knowledge and ability to demonstrate insulin delivery devices and testing devices and able to troubleshoot.

Works closely with other health professionals to assure continuum of care, specifically regarding diabetic information.

Utilizes current ADA content standards as an overall guideline to excellent Diabetic education processes.

Works to implement diabetes education programs and improve existing programs.

Documents assessments and interventions in medical records per hospital guidelines and in compliance with ADA Program.

Documentation is completed in a timely manner.

Displays excellent customer services skills through creative problem solving and strong verbal communication skills

Able to work in a team environment and collaborate with others within the department / health system as well as in the community.

Must be able to work efficiently and responsibly in an environment with multiple work demands and short time frames using good time management skills

Ability to work independently with limited supervision

Maintain the privacy of all patients

Act as community resource/consultant when requested.

Provides a therapeutic presence in service to others by purposefully responding to the needs of patients/customers in a caring way. It includes introducing oneself and stating your purpose, asking the patient their preferred name, sitting with the patient to determine their care goals, active listening, and the skillful use of communication skills (touch, eye contact, etc.).

Calculates dosages and administers medications accurately in accordance with SCHS policies and procedures for medication administration.

Provides and maintains a safe environment for caregivers, patients and guests.

Supports the vision, mission and values of the organization in all respects.

Documents all patient care with proficiency in compliance with hospital policies and regulatory agencies.

Role models two-way communication of the SCHS mission, vision, values, and strategic plan for the department.

Conducts all activities with the highest standards of professionalism and confidentiality. Complies with all applicable laws, regulations, policies and procedures, supporting the health system's corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all company investigations and proceedings.

Delivers customer service and/or patient care in a manner that promotes goodwill, is timely, efficient and accurate.

May perform additional duties of similar complexity within the organization, as required or assigned.

EDUCATION

Required: Bachelor's degree in Nutrition, Dietetics or related field.

Preferred: Master's degree in a related field

LICENSURE/CERTIFICATION/REGISTRATION

Required: Registered Dietitian from the Commission on Dietetic Registration.

Certified Diabetes Educator (CDE) certification from the National Certification Board for Diabetes Educators (NCBDE)

Valid Oregon driver's license and ability to meet SCHS driving requirements. Ability to travel to business functions/trainings/meetings and all SCHS worksites required.

Preferred: Current Oregon dietitian license.

AHA Basic Life Support for Healthcare Provider certification

EXPERIENCE

Required: 3 years patient care experience working directly with patients in helping to manage their diabetes.

Preferred: N/A

PERSONAL PROTECTIVE EQUIPMENT

Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

Computer

Basic to intermediate ability and experience in computer applications, specifically electronic medical records system and MS Office.

Basic experience in computer applications necessary to record time, obtain work directions, and complete assigned CBL's.

Intermediate to advanced proficiency in Microsoft applications (Outlook, Word, Excel and Access), database management, and document preparation.

Demonstrated ability and experience in computer applications, use of electronic medical record keeping systems and MS Office.

PHYSICAL REQUIREMENTS:

Continually (75% or more): Use of clear and audible speaking voice and the ability to hear normal speech level.

Frequently (50%): Sitting, keyboard operation.

Occasionally (25%): Standing, walking, bending, stooping/kneeling/crouching, climbing stairs, reaching overhead, lifting/carrying/pushing or pulling 1-10 pounds, grasping/squeezing, operation of a motor vehicle, ability to hear whispered speech level.

Never (0%): Climbing ladder/step-stool, lifting/carrying/pushing or pulling 11-50 pounds.

Exposure to Elemental Factors

Occasionally (25%): Chemical solution for Laboratory Caregivers.

Rarely (10%): Wet/slippery area.

Never (0%): Heat, cold, noise, dust, vibration, chemical solution, uneven surface.
