Community Bridges is seeking a Direct Support Professional (DSP) to join our team in Deering, NH.

Founded in 1982, Community Bridges is a nonprofit organization dedicated to advancing the integration, growth, and interdependence of individuals with developmental disabilities within their own communities. Serving more than 1,100 individuals and families across the Greater Capital Area, we work to remove barriers and create meaningful community engagement.

Position Overview

Direct Support Professionals support individuals who experience developmental disabilities and/or have medical needs requiring consistent attention throughout the day, along with occasional behavioral challenges. Individuals follow regular daily routines, enjoy community activities, and are eager to learn and explore new experiences.

DSPs provide hands-on support with health, safety, and daily living while fostering independence, dignity, and meaningful community connections.

Qualifications

  • Education: High School Diploma or GED required

  • Experience: Experience in human services or a related field supporting individuals with disabilities preferred

  • Reliable transportation, valid driver's license, and proof of automobile insurance required

Compensation & Benefits

  • $1,000 Sign-On Bonus

    • $500 paid at hire

    • $500 paid after 6 months of employmentSign-on bonus available to new hires only. Re-hires and transfers are not eligible. Bonus for full time employees only.

Key Duties & Responsibilities

  • Assist individuals with planned activities and community involvement

  • Develop relationship maps to identify social inclusion opportunities

  • Support individuals in building meaningful personal and community relationships

  • Promote self-esteem through active participation in the community

  • Provide individualized supports including personal care and skill acquisition

  • Assist with hygiene, bathing, toileting, and dental care as needed

  • Assist with person-centered planning and implementation of goals

  • Complete required documentation, data collection, and progress notes

  • Assist in orientation and training of new staff

  • Administer medications in accordance with He-M 1201, as applicable

  • Report safety, health, or human rights concerns immediately

  • Ensure compliance with applicable state regulations and agency policies

  • Perform additional duties as assigned

Required Skills

  • Strong verbal and written communication skills

  • Ability to meet documentation deadlines

  • Ability to work independently with sound judgment

  • Ability to support daily living and functional skill development

  • Ability to utilize community resources and opportunities

  • Ability to work collaboratively as part of a team

  • Basic computer skills including email and word processing

  • Positive, professional attitude in stressful situations

Work Environment & Physical Requirements

  • Primarily indoor day program setting with occasional outdoor activities

  • Minimal noise levels

  • Ability to lift up to 50 lbs. and assist with transfers

  • Frequent walking, standing, and occasional stair climbing

  • Ability to operate wheelchairs and mechanical lifts

  • Emergency behavioral interventions may be required per individual plans

Travel Requirements

  • Occasional use of a personal vehicle to transport supported individuals

  • Valid NH driver's license and acceptable driving record required
