Job Description
A Director of Case Management opportunity in North Richland Hills, TX (northeast Tarrant County) at a 165-bed acute care hospital designated as a Level III Trauma Center, Primary Stroke Center, and accredited Chest Pain Center, with comprehensive cardiovascular services, minimally invasive robotic surgery, spine and joint programs, a geriatric behavioral unit, and a women's imaging center.The role. The Director of Case Management holds full operational accountability for the hospital's case management department, leading a team of approximately 10 FTEs and 7 PRN staff while reporting directly to senior leadership and working closely with the CMO and Medical Director. The Director oversees the full case management model including staffing, skill mix, onboarding, and staff competencies while ensuring timely, well-documented care coordination and discharge planning that supports continuity of care across interdisciplinary teams and external agencies. A core focus of this role is driving clinical resource utilization outcomes by analyzing operational data, implementing sustainable performance improvement initiatives, and partnering with the facility CFO and Division Case Management Leader to meet established goals. The Director cultivates strong working relationships with key stakeholders including the CMO, CNO, ancillary service leaders, and Ethics and Compliance, serving as the department's subject matter expert and executive voice in system-wide case management initiatives. Some on-call rotation and occasional extended hours may be required.What we're looking for.
- Bachelor's degree required
- Master's degree in Nursing, Health Administration, or Business Administration preferred
- Active Registered Nurse (RN) licensure, Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) required
- 3+ years of acute care hospital case management experience required
- 2+ years of case management leadership experience required
