Job Overview

We are seeking an experienced Director of Case Management to oversee hospital utilization management, transition management, and operational leadership within the Case Management Department. This role is responsible for promoting effective utilization of hospital resources, supporting timely patient throughput, improving revenue cycle processes, and ensuring compliance with state and federal regulations.

The Director will collaborate closely with interdisciplinary teams, physicians, and leadership to enhance care coordination, denial prevention, patient satisfaction, and operational efficiency across the organization.

Key Responsibilities

• Oversee daily operations of the Case Management Department• Lead utilization management and hospital throughput initiatives• Ensure compliance with CMS regulations and clinical policies• Support denial prevention and revenue cycle optimization• Coordinate patient transition planning and discharge processes• Provide physician education related to hospital utilization practices• Monitor operational performance metrics and quality outcomes• Collaborate with interdisciplinary healthcare leadership teams• Promote patient-centered care coordination and regulatory compliance• Support continuous process improvement initiatives

Requirements

Bachelor's degree in Business, Nursing, or Healthcare Administration for RN candidates, or Master's in Social Work for MSW candidates Active Registered Nurse (RN) or LCSW/LMSW license in applicable state(s)
