Direct all Case Management department operations for a 50-bed inpatient rehabilitation hospitalLead, mentor, and develop a team of Case Managers and Case Management support staffOversee utilization review, discharge planning, care coordination, and patient progression activitiesEnsure compliance with hospital policies, regulatory standards, and best practicesDevelop strategies to maximize quality outcomes and efficient patient transitions of careMonitor departmental performance metrics and implement process improvementsCollaborate closely with physicians, nursing leadership, therapists, and interdisciplinary teamsSupport case management staff during high-volume periods and assist with patient coverage when neededFoster a positive team culture focused on accountability, patient experience, and operational excellenceWork directly with executive leadership to achieve organizational goals and performance objectivesSupport certification and professional development initiatives within the department
