Lead all Case Management department operations within the hospitalOversee utilization review, discharge planning, and care coordination activitiesManage and mentor a team of Case ManagersRecruit, onboard, and develop new Case Management staffSupport improvements in patient experience and discharge planning outcomesCollaborate with physicians, nursing leaders, therapists, and interdisciplinary teamsMonitor departmental performance metrics and implement process improvementsEnsure compliance with regulatory and accreditation requirementsAssist with utilization management initiatives and resource optimizationCarry a patient caseload as needed while supporting department operationsFoster a positive culture and high-performing Case Management environmentPartner with executive leadership to achieve organizational goals
