The Role Itself

  • Works collaboratively with physicians and hospital staff to assure completion of records that meet all regulatory and facility requirements.

  • Oversees the coding process to assure that it is done timely and accurate within 72 hours of discharge.

  • Answer inquiries from internal and external contacts regarding information recorded in patient’s medical records, within all departmental policies and guidelines.

  • Notify supervisor regarding any potential problems with record updates, maintenance and/or breaches of confidentiality.

  • Performs timely audits on medical charts to ensure documentation is completed accurately.

  • Prepares/audits the medical records post discharge to assure all required documentation is present.

  • Accurately records and monitors departmental signature deficiency rate and maintains rate below 10%.

  • Informs/educates all facility staff of HIPAA policies pertaining to medical records, and remain current on all laws and regulations regarding confidentiality of patient/medical information.

  • Provides leadership and management to all Medical Records staff and conducts regular departmental meetings to consistently improve service.

  • Adheres to release of patient information/medical records according to hospital and regulatory requirements.

  • Ensures dictation reports are completed and filed in the MR according to time requirements.
