Director of Nursing 

Region: Southern Colorado, ideal candidate is located south of Colorado Springs and comfortable with occasional travel to Pueblo

Employment Type: Full-time, Mon-Fri

Compensation: $90,000-$110,000

Position Summary:

The Director of Nursing (DON) is responsible for the overall clinical operations of the home health agency, ensuring high-quality patient care, regulatory compliance, and effective leadership of the clinical team. The DON oversees daily clinical functions, supervises nursing and therapy staff, manages clinical workflow, and ensures adherence to state, federal, and accreditation standards. This role is essential in supporting organizational growth, maintaining quality outcomes, and fostering a culture of excellence. 

Essential Duties & Responsibilities:

Clinical Leadership 

  • Provides oversight and direction to all clinical staff, including RNs, LPNs, therapists, and aides. 

  • Ensures timely and accurate completion of patient assessments, OASIS documentation, and Plan of Care development. 

  • Leads weekly case conferences and interdisciplinary team meetings. 

  • Monitors patient outcomes, satisfaction, and quality improvement metrics. 

  • Ensures appropriate patient assignments and caseload management. 

Regulatory & Compliance Oversight 

  • Ensures compliance with Medicare Conditions of Participation, state home health regulations, and accreditation standards. 

  • Oversees chart audits, clinical documentation review, and quality assurance activities. 

  • Ensures timely follow-up on ADRs, surveys, and corrective action plans. 

  • Maintains up-to-date knowledge of home health regulations and industry best practices. 

Staff Management & Development 

  • Recruits, trains, evaluates, and mentors clinical staff. 

  • Conducts performance reviews and supports professional development. 

  • Addresses staffing needs, oversees scheduling, and ensures adequate coverage. 

  • Provides coaching, corrective action, or disciplinary measures when necessary. 

Patient Care Oversight 

  • Ensures high-quality, patient-centered care rooted in best clinical practices. 

  • Participates in or oversees Start of Care (SOC), Recertification, and supervisory visits when needed. 

  • Reviews incident reports and ensures timely reporting and follow-up. 

  • Responds promptly to escalated patient or family concerns. 

Operational & Administrative Responsibilities 

  • Collaborates with the VP of Clinical Services to develop and implement clinical policies, procedures, and workflows. 

  • Participates in budget planning, staffing models, and resource management. 

  • Reviews clinical reports and data to identify operational improvements. 

  • Supports onboarding and orientation of new clinicians. 

Minimum Qualifications:

  • Active RN license in the state of practice (unrestricted). 

  • Minimum 3–5 years of nursing experience, preferably in home health or a related field. 

  • Minimum 2 years in a clinical leadership or supervisory role preferred.  

  • Strong knowledge of Medicare home health regulations, OASIS, and electronic medical records. 

Required Skills & Competencies:

  • Strong leadership and team-building abilities. 

  • Excellent communication, organization, and problem-solving skills. 

  • Ability to manage multiple priorities in a fast-paced environment. 

  • High level of clinical judgment and critical thinking. 

  • Competence with EMR systems, OASIS documentation, and digital communication tools. 

  • Professionalism, integrity, and a commitment to quality care.
